The process

Three steps, no runaround.

You send the basics

County, program, current agency, and what isn’t working. Nothing that identifies the individual — we’ll gather that on the call.

A nurse calls you back

Within 24 hours. We talk through timing, overlap, and whether the current DSP can come across too.

We plan the handover

No gap in coverage. Where the family wants continuity and the DSP is willing, we work to bring that person with them.

Request a transfer

No names. No clinical detail.

This form is deliberately free of protected health information. Tell us the county, the program, and what isn’t working now — a nurse collects the rest on the call. The plan moves in iRecord: update the provider on the ISP and it follows the individual across to us.

Please do not enter protected health information. No names, dates of birth, Medicaid or DDD ID numbers, diagnoses, or medical detail. Use your own case reference below. Once we accept, the plan moves to us through iRecord — you never need to email it.

High level only. No incident detail or clinical reasons — we’ll discuss on the call.
If a DSP is willing to move with the individual, we will try to make that work.
Please leave out names, dates of birth, ID numbers, diagnoses, and medical detail.

Moving the plan

How the ISP follows the individual across to us.
The plan moves in iRecord. Don't email it.
On a transfer, the ISP follows the individual when the provider is updated in iRecord. Once Tender Hand is added we pull the plan, service detail and budget from there. To add us you'll need:
