The process

Three steps, no runaround.

You send the basics

Discharge date, county, and services needed. Nothing that identifies the individual — we’ll gather that on the call.

A nurse calls you back

Within 24 hours, and the same day when discharge is pending. We confirm fit, capacity, and timeline honestly — including when we’re not the right agency.

We meet the individual before day one

Where we can, we meet on the unit or at home before discharge, so the DSP isn’t a stranger on the first day home.

No names. No clinical detail.

This form is deliberately free of protected health information. Give us the discharge date, county, and what the person will need at home — a nurse collects the rest on the call. Please don’t send clinical records: where there is a Support Coordinator, we take the plan through iRecord with them.

Please do not enter protected health information. No names, dates of birth, medical record or Medicaid numbers, diagnoses, or clinical detail. Use your own case reference below — a nurse will collect everything else on the call.

The discharge

What we need to move fast — without identifying anyone.
Use your internal reference. Please do not enter the individual’s name or MRN.
Practical detail only. Please leave out names, dates of birth, ID numbers, diagnoses, and medical detail.
Please don't send clinical records.
If the individual has a DDD Support Coordinator, the plan reaches us through iRecord once Tender Hand is added as the provider — tell us who the coordinator is above and we will take it from there. If there is no coordinator yet, we will walk you through it on the call.

Ordinary email is not a secure channel for protected health information.
