The process

Three steps, no runaround.

You send the basics

County, program, services, and hours. Nothing that identifies the individual — we’ll gather that on the call.

A nurse calls you back

Within 24 hours. We confirm fit, capacity, and timeline honestly — including when we’re not the right agency.

We meet the individual and family

We match a DSP to the person, not to an open shift. You get a named point of contact for the case.

Make a referral

No names. No clinical detail.

This form is deliberately free of protected health information. Give us your case reference, county, program and service needs — a nurse collects the rest on the call. The plan itself never travels by email: add Tender Hand in iRecord and we pull the ISP, service detail and budget from there.

Please do not enter protected health information. No names, dates of birth, Medicaid or DDD ID numbers, diagnoses, or medical detail. Use your own case reference below. Once we accept, the plan reaches us through iRecord — you never need to email it.

How the ISP reaches us once we accept.
Add us in iRecord. Don't email the plan.
Once Tender Hand is added as the provider, we pull the ISP, service detail and budget straight from iRecord. To add us you'll need:
